I understand that The Tressing Room provides cosmetic hair and scalp care, professional observations, and hair care education within the scope of cosmetology. An Individual Hair Care Plan is educational and is not medical advice, diagnosis, or treatment. I understand that concerns outside the scope of cosmetology may require evaluation by my child's pediatrician, dermatologist, or another qualified healthcare professional.

Information Acknowledgment *

I understand that recommendations may be based on the information and photos I provide, along with relevant observations from my child's Tressing Room appointments when applicable. I have provided information that is accurate to the best of my knowledge.

Turnaround & Delivery Acknowledgment

I understand that submitting this form does not begin the Hair Care Plan preparation period. If my request is approved, I will receive the appropriate payment link. New Individualized Hair Care Plans and Consultation Add-On Plans are delivered within 5 business days after payment, and Hair Care Plan Refreshes are delivered within 3 business days after payment.
